Shockwave Therapy vs PRP (P-Shot) vs Stem Cell Therapy for ED: How They Compare

Shockwave therapy, PRP (the “P-Shot”), and stem cell therapy get grouped together a lot — all three are non-surgical, regenerative-style approaches to ED that don’t involve daily medication. But they work differently, have very different amounts of research behind them, and suit different situations. Here’s an honest side-by-side.

Shockwave therapy (ESWT)

How it works: Low-intensity sound waves delivered through a handheld device stimulate new blood vessel growth in penile tissue, aiming to improve blood flow.

Procedure: Non-invasive, no anesthesia, no needles. A typical course is around six sessions, roughly weekly, 15–20 minutes each.

Evidence: The most independently reviewed of the three. A 2019 Cochrane review of 21 studies (1,357 men) found a modest effect on erectile function, more likely to be noticeable over the longer term than the short term. Still considered experimental and not FDA-approved for ED.

Safety: Generally well tolerated — clinical sources describe side effects as rare and mild (temporary discomfort, minor bruising).

PRP / P-Shot

How it works: Platelet-rich plasma, separated from a sample of your own blood, is injected into penile tissue with the aim of stimulating tissue repair and improved blood flow.

Procedure: Involves a blood draw and injection — more invasive than ESWT.

Evidence: Thinner than ESWT’s evidence base. It’s grouped by U.S. hospital sources alongside shockwave and stem cell therapy as an experimental option “not yet approved by the FDA,” with long-term effectiveness and safety not well established.

Safety: As an injection procedure, it carries the general risks of any injection (bruising, discomfort at the injection site), on top of the same uncertainty about long-term outcomes as other regenerative options.

Stem cell therapy

How it works: Aims to use stem cells to help restore or regenerate erectile tissue and function.

Procedure: More invasive and technically involved than ESWT.

Evidence: By far the earliest-stage of the three. Much of the human data cited by hospital sources comes from small, specific patient groups — for example, limited testing in men who’d had their prostate surgically removed — not the general ED population. The Sexual Medicine Society of North America has described the underlying science as showing promise in animal studies while explicitly cautioning that patients considering it should be “fully informed and consented regarding the potential benefits and risks,” given how few human clinical trials exist.

Safety: The least established of the three, precisely because there’s the least clinical trial data to establish it from.

So which one is “better”?

There isn’t a single right answer — it depends on the cause and severity of your ED, what you’ve already tried, your comfort with a more invasive procedure, and how much weight you put on having a larger, more independently reviewed evidence base versus a newer approach. ESWT currently has the most research behind it of the three. PRP and stem cell therapy are options some men explore, generally with a clearer understanding going in that the evidence is still developing.

This is exactly the kind of decision worth making with a doctor rather than by comparison-shopping online — particularly since some men are reasonable candidates for more than one option, or none of them, depending on what’s actually causing their ED.
